Older Adult Community Assessment Table Part 1

This is the scenario: someone in your neighborhood is a 78-year-old female, Mrs. E., and lives alone with her dog in a home she owns. She takes the dog for walks and considers these walks her exercise. Mrs. E. enjoys getting out with people her age to socialize. She is on medication for mild hypertension but is otherwise healthy. Mrs. E. is on a fixed income, social security, and a small retirement from the job she retired for a total of $1750 per month. She tries very hard to watch her spending but must pay for utilities, food, gas, and co-pays for physician visits. She has a son, but he works full-time and is frequently out of town on business. His wife is very busy with work and taking care of their 3 children. Mrs. E. still drives but only on side streets as she is no longer comfortable on the freeways and does not drive at night. 

1. The nearest Pharmacy – Do they compound medications? Do they deliver? Is there a pharmacy in the area that delivers or compound medication? 

1. The nearest grocery store where the older adult could get fresh fruit and vegetables. Do they deliver? Does any grocery in the area delivers and what will the costs be to Mrs. E?

1. If Mrs. E couldn’t drive what resources are available for her to meet her needs considering her limited income?  

1. Nearest senior center or other facilities with activities for seniors. Do they provide transportation? Do they serve lunch? What will this cost Mrs. E?

10. Is there a meal delivery service in the area that OA such as Mrs. E could afford?   

11. Are there sidewalks in the neighborhood? Are they in good condition? Would they

be a fall risk? 

12. Is there a park nearby to walk the dog? Would she have to

walk in the street? How safe would that be?

14. Nearest pet supply store.

15. What is the crime rate in the area? 

16. The nearest fire station – police station – EMT?

17. Can she afford to obtain lawn service available in the area?

18. Is there someone nearby for house cleaning that she can afford?

Create a detailed table (given in instructions) with the information gathered from the above questions.
